Please inform the receptionist if you change your name, address or telephone number. We must have your telephone number even if it is ex-directory so we can contact you in an emergency. It will be kept strictly confidential. Please telephone the reception team on 0115 963 3511 Option 5 between 10am - 6.30 pm.

If you move out of the practice area, you will be required to find a new doctor.
