How To Register

Thank you for choosing this practice. Oakenhall Medical Practice welcomes all patients living in Hucknall, Linby, Bestwood Village and Papplewick (for those patients residing in Papplewick, please telephone the practice to verify that you are living in the practice boundary area).

To register please complete the Health Questionnaire and Registration Form (the forms are also available to collect from the surgery) for each member of your household and return these to the practice with two forms of identification - Photo ID (Passport, Driving Licence) and Proof of residency (Utility Bill, Bank Statement) and your NHS Number.

When you return the documentation you will be offerered an appointment for the Healthcare Assistant . The Healthcare Assistant offers new registration checks for all new patients which includes a brief health check and advice.

Following your registration, your computer held records may be transferred to the practice electronically. Your paper held medical records will be forwarded to the practice by Patient Services.

If you would like to register for online service's for booking appointments and requesting of prescriptions, please complete the Online Services Application Form (located on the Online Services website page). Identification (as above) will be needed for this purpose and return it with your registration forms.

Oakenhall Medical Practice is committed to providing quality care and in order to achieve this, the GP's recommend that your computer medical record is shared with other NHS Professionals, i.e. Health Visitors, 111, Emergency Services, District Nurses. We would like to assure you that this access is restricted and monitored to the NHS professionals only who are involved in your healthcare. Please note all computer patient records will automatically be shared, but would appreciate your explicit consent to this. If you have any questions or concerns please speak to your GP at your next routine appointment.

Transfer of Electronic Patient Information

NHS England continues to develop the facility of transferring patient information electronically within GP Practices. This includes:

  • Transfer of Patient Records between practices, when a patient registers or de-registers with a GP Practice
  • The facility to enable patient to view online, export or print any summary information from their records i.e. medication, allergies, advice reactions and any additional information agreed between the GP and the Patient.
  • The Summary Care Record enables healthcare staff faster and easier access to essential information about you i.e. medication, allergies and adverse reactions. This information will only be viewed in an emergency or in urgent care settings with the explicit consent of the patient unless the patient is unable to give their consent e.g. if they are unconscious.

Named Accountable GP for all Patients
All patients have a named GP who is responsible for patient's overall care at the practice. This named GP will take responsibility for the coordination of all services that are required by you. If you wish to know your named GP, please contact the practice. If you have a preference for a particular named GP, please contact the practice to discuss this and the practice will make reasonable effors to accommodate your request.
